The Department of Health at provincial level — or, after the 2024–2025 local-government restructuring, the equivalent specialised health authority under the provincial People's Committee. Sở Y tế issues the GCN ĐĐKKDD for retail pharmacy establishments (nhà thuốc, quầy thuốc, tủ thuốc trạm y tế xã, traditional retail), the CCHND (Chứng chỉ hành nghề dược) for individuals, the số lưu hành for Class A/B medical devices via self-declaration of conformity, and conducts provincial-level inspection and enforcement.
